If a TMA student is requesting to bring a friend who is not a current TMA student, they must have this form completed before purchasing a ticket to the dance. This form must be filled out and turned in to Mr. Stomprud’s office at least two days prior to purchasing a ticket to the event. This form must verify that the guest is a current high school or college/university student and is under the age of 21. In addition, the guest must present his/her photo ID at the door the evening of the dance.
