Sports Physicals at TMA Campus

TMA will be offering sports physicals on August 3rd and 4th at The Master’s Academy for $15.00.

You must call the Upper School office and set up an appointment in advance because there are a limited number of spots available.

Dr. Kent Hoffman will be administering physicals on August 3rd from 7-9 pm.
Dr. Perri Dumbacher will be administering physicals on August 4th from 9am-5 pm.

Parents must fill out the FHSAA Pre-participation Physical Evaluation Form in advance and have it at the time of the physical and must also remain with their child throughout the physical. All forms can be found on the TMA website. Please contact the Upper school office to schedule an appointment.
